If your child is a DentaQuest Medicaid member and received care on or after February 1, 2025, you may qualify for one of the following:
- For DentaQuest Medicaid members who have an extraction.
- Member must watch opioid risk awareness video* and take a quiz.
- Adult Medicaid Members 21 and older.
- One per Member per calendar year.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.
- As part of our Smiling Stork program and for DentaQuest Medicaid Members who have a preventive dental visit during their pregnancy.
- Member must watch a video on how to prevent early childhood caries* and take a quick quiz.
- Adult and child Medicaid Members who are pregnant.
- One per Member per calendar year.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.
- For DentaQuest Medicaid Members who receive at least one sealant on a permanent molar.
- Member must watch our "Teeth Tips" video.*
- Child Medicaid Members 9 and under.
- One per Member per calendar year.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.
- For DentaQuest Medicaid Members who receive at least one sealant on a permanent molar.
- Member must watch our "Teeth Tips" video.*
- Child Medicaid Members ages 10‐14.
- One per Member per lifetime.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.
- For DentaQuest Medicaid child Members who have an oral evaluation or preventive treatment within 180 days of enrollment.
- Member must take an online oral health quiz.
- Child Medicaid Members ages 0‐20.
- One per Member per calendar year.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.
- For DentaQuest Medicaid Members who complete a follow-up visit with their Main Dentist within 30 of an emergency room visit for a dental-related issue.
- Member must take a quiz about emergency room dental visits.
- Medicaid Members of any age.
- One per Member per calendar year.
- Member may earn other Healthy Behaviors rewards; the total amount of all rewards earned cannot be more than $50 in the calendar year.

Fill out the form below and submit it to DentaQuest.
The gift card can be used to purchase:
- Oral health supplies such as toothbrushes, toothpaste, floss and mouthwash.
- Baby care items such as diapers, bottles and formula.
- School supplies and clothes.
Forms must be submitted to DentaQuest within 60 days of the date of service. Treatments must be completed by an in-network provider while the Member is enrolled with DentaQuest.
When can you expect your gift card?
If you qualify, you can expect to receive your reward 6-8 weeks after you submit the form.
